Building a connection that lasts requires thoughtful attention to strengths, interests, goals, personality, communication styles, relationship styles, preferences, lifestyle and support needs.
When these elements are considered together, the connection process becomes intentional and more likely to support stability over time.
Our approach to connection
Soaring Connections was created to bring structure and clarity to the process of finding self-directed support. We believe that stronger connections are formed when both individuals and providers move forward with shared understanding and realistic expectations.
Our focus is to help individuals and guardians identify what matters most in their support relationships, and to help providers present their experience and approach clearly. This allows both sides to explore connection with purpose and confidence.
Our role in the connection process
Soaring Connections helps make the connection process more accessible and easier to navigate. We gather and organize information, develop clear profiles and create visibility for open connections. We guide the process so individuals, guardians and providers move forward with clarity and realistic expectations.
Individuals and guardians direct services and make final decisions. Providers determine their availability, scope and approach to support. Our role is to facilitate introductions and provide structure so both sides can begin with shared understanding.
Our hope is to support individuals and providers in forming intentional connections that lead to meaningful supports, stronger relationships and long-term stability.
Participation in the connection process does not require public visibility. Profiles are shared publicly only with approval.
Meaningful connections begin with clear information. When individuals and providers understand one another’s experience, interests and approach to support, they are better able to explore connections that may lead to strong and lasting support relationships.
Provider information and introductions
When providers choose to pursue connection through Soaring Connections, they are invited to share a short provider profile. This information helps individuals and guardians learn more about potential providers before deciding whether they would like to move forward with a conversation.
Provider profiles may include a provider number, a summary of experience supporting individuals with I/DD, professional references and a brief personal introduction. Many providers also choose to share a little about their interests, the types of support they enjoy providing and what led them into this work.
Sharing this information allows individuals and families to understand both the professional background and the personal approach of a potential provider. This supports informed decision-making and helps both sides explore connections with greater clarity.
Soaring Connections gathers and organizes this information so introductions can be facilitated thoughtfully and transparently. Our role is to help individuals and providers learn about one another and determine whether a connection feels like a good fit.
Provider participation within the Soaring Connections community reflects a shared commitment to respectful communication, transparency and relationship-centered support.
How the connection process works

Submit a referral inquiry to begin.

We review the information and follow up to clarify goals, preferences and next steps.

A profile is developed and approved before being shared.

Open connections are posted for visibility on our website and shared through communication channels.

Individuals, guardians and providers may express interest in a connection. When interest is expressed, we share relevant information and facilitate an introduction. Introductions move forward based on the direction and consent of the individual or guardian.
Each step is designed to support informed choice, clarity and steady beginnings.
