For individuals and families, referrals provide visibility and access to providers who are open to new connections. For providers, they create a pathway to share their strengths and reach those who are already looking for support. Referrals bring people together through shared purpose and a commitment to meaningful connection.
How the Referral Process Works
Referrals can be submitted by:
• Individuals or families seeking an independent provider
• Independent providers interested in connecting with individuals
• Service coordinators assisting individuals in their search for a provider
Privacy, Verification and Background
Soaring Connections values and protects the privacy of everyone who participates in the referral process. Personal contact information is never shared publicly, and referrals are shared only through Soaring Connections’ communication channels.
To support safety and accountability, we complete a basic verification for all providers. This includes confirming the provider’s number and active status with DHHS, reviewing resumes, and contacting references. Providers with a verified provider number have already completed required background and registry checks through DHHS as a part of enrollment. The role of Soaring Connections focuses on verifying provider status and references to ensure information is accurate and current.
Providers are not fully verified or endorsed by Soaring Connections unless they have completed the Soaring Connections Certification Pathway. Individuals, families, and providers are encouraged to continue their own background and compatibility checks prior to beginning services.Referral listings may be edited or removed at any time.
